When you first walk through our doors, you can expect to be welcomed with friendly smiles from a team of health care professionals who care about you.
Step One: Paperwork
In order to provide you with the highest quality of care, and to insure that we have all the information your insurance company requires, we’ll ask you to complete confidential patient history and health history forms. This step provides us with general information about yourself and your condition. You can fill these forms out at our clinic or download the patient forms from below, and complete them in the comfort of home so you can save yourself some time on the day of your first appointment. When scheduling your appointment – please let us know that you will be bringing the paperwork with you. Click on the icons below to download the forms, then read and sign them.

Step Two: Your First Visit
A one-on-one consultation and examination with a doctor will be done to discuss your health issues and goals, uncover the layers of past damage done and to help determine what may be the cause. Please understand that you will probably not receive an adjustment on your first visit. It is important that we gather all pertinent information and perform necessary diagnostic tests before starting treatment.
Your Initial Consultation
The doctor will meet with you to discuss your health problems, concerns and potential treatment options. The more we know about you, your health history and your current condition, the better we can help you restore your quality of life.
Examination
A complete chiropractic examination will be performed that involves testing your reflexes, flexibility and other standard neurological, orthopedic, postural, and physical examinations.
X-Rays
X-rays assist us in developing the most effective treatment plan for you, and aid in ruling out more serious conditions. Your specific condition may require you to have a series of x-rays taken to help identify misalignments or degeneration in the spine so an effective treatment plan can be implemented.
Step Three: Report of Findings Visit
You will be asked to schedule a special follow-up appointment called the “Report of Findings” at which time the cause of your current condition will be discussed. Once all of the information has been gathered and examinations have been performed, Dr. Crist will give you his report of findings. After reviewing your case, the doctor will decide if you will be accepted as a patient and a thorough explanation of care recommendations and the action steps you need to take to obtain the results you desire. During this report you will know exactly how we can help you. Plus, you’ll receive specific recommendations about what you can do to help yourself get better as well as the costs associated with care. You will be given the option to initiate care and receive an adjustment at this point.
